What is a Slip Disc?

A slipped disc — medically known as a herniated disc or Prolapsed Intervertebral Disc (PIVD) — occurs when the soft inner core of a spinal disc (nucleus pulposus) pushes through a crack in the tough outer cartilage (annulus fibrosus) and presses on nearby nerves. This is one of the most common causes of severe back and leg pain.

Slip disc most commonly occurs in the lumbar spine (L4-L5 and L5-S1 levels) causing lower back pain and sciatica. It can also occur in the cervical spine (neck) causing arm pain and weakness.

Important: A "slipped" disc doesn't literally slip out of place — it bulges or herniates. The severity ranges from a small bulge (contained herniation) to a large extrusion compressing the spinal cord or nerve roots.

Diagnosis

  • MRI scan — definitive diagnosis of disc herniation and nerve compression

  • Straight leg raise test — assesses nerve root irritation
  • CT myelogram if MRI is contraindicated

Conservative Care (First Line)

90% of slip disc patients improve with physiotherapy, NSAIDs, nerve pain medications, hot/cold therapy, and activity modification over 6–8 weeks.

Epidural Steroid Injection

Targeted injection of corticosteroid and local anaesthetic around the affected nerve provides significant pain relief and enables physiotherapy.

Endoscopic Discectomy

Keyhole surgery through a 7mm incision to remove the herniated disc fragment. Day care procedure — walk the same day, home next morning.

Can a slip disc heal without surgery?
Yes. In approximately 90% of cases, slip disc heals naturally over 6–12 weeks with conservative treatment including physiotherapy, pain medications, and lifestyle modification. The herniated disc material is gradually reabsorbed by the body. Surgery is needed only when neurological symptoms are progressive or when conservative treatment fails.
Is endoscopic discectomy better than microdiscectomy?
Both procedures have excellent outcomes with 90–95% success rates. Endoscopic discectomy uses a 7mm incision (vs 3–4cm for microdiscectomy), local anaesthesia, and allows same-day recovery.
